Specialist Health Care and Personal Care Services

  • Third Party Support for Long Term Care Homes, Retirement Facilities, and Home Healthcare Agencies Access provides staff coverage due to illness, job vacancy, training and/or vacation to ensure support is provided to clients on a consistent basis. The Management team at Access works collaboratively with agencies to meet growth staffing expectations, while ensuring all client-centred care exceeds the current standards of practice.
  • Motor Vehicle Accident Services for Acquired Brain Injury, Spinal Injury and Challenging Behaviours Access has a core team of specialized Nurses and Support Workers with experience in managing the unique needs of survivors of acquired brain injury, spinal injury, emotional and behavioural issues.
  • Post-Operative and Rehabilitation When your loved one is recovering from surgery our Nurses and PSWs can assist with wound care, physical transfers, ADLs, day-to-day responsibilities and physiotherapy programs (range of motion and flexibility).
  • Community and Residential Support APS offers Nurses and Personal Support in residences, hospitals, rehabilitation facilities, long-term care, retirement homes, supportive housing and mental health centers.
  • Household Management When a family member becomes mentally or physically incapable of contributing to household responsibilities, our PSWs are available to help with meal preparation, housekeeping, laundry and the many organizational tasks associated with home management.
  • 24-Hour and Overnight Support PSWs provide 24-hour care service aimed at supporting individuals who require a high level of support throughout the day and night. Night coverage is available. Duties could include but are not limited to; repositioning the client, toileting, laundry, cleaning and food preparation as necessary.
  • Caregiver Relief When a family member is responsible for their loved one's care, it can become physically and emotionally demanding. APS provides families a well-deserved break to alleviate stress and minimize the chance of burn out.
  • Emergency Back-up Support APS offers round-the-clock Personal Support. Call us anytime and we will coordinate emergency services in a quick and timely manner.
  • Passive Night Support Our PSWs provide passive night support at a reduced rate, in the event that a sleep shift is required. The PSW is readily available for assistance with periodic needs such as toileting, in case of a medical emergency, and/or to ensure your loved one is safe throughout the night.
  • Senior Care and Disability Companion care services are offered when your loved one feels the loss of independence due to aging or disability. You can have peace of mind that our Nurses and PSWs will provide the highest level of care to ensure they are in good hands.
  • Dementia Care Nurses and PSWs accommodate every client's unique needs, preferences and strengths to promote independence, self-esteem and allow for the highest quality of life possible.
  • Palliative Support Our Nurses and PSWs provide palliative support in all stages of care, when family members are undergoing treatment for illnesses, chronic ailments, as well as those who are nearing the end of life. Focus is placed on quality of life and increasing comfort.
  • Personal Care PSWs provide assistance for activities of daily living, such as showering, shaving, toileting, dressing, grooming and meal preparation.
  • Foot Care Problems with feet can be the first sign of more serious medical conditions such as arthritis, diabetes, and nerve and circulatory disorders. Our Nurses and PSWs provide foot care to facilitate increased circulation, improve foot hygiene and minimize the risk of further issues.
  • Respite Respite care provides temporary relief for a primary caregiver, enabling you to take a much-needed break from the demands of caregiving a sick, aging, or disabled family member. Respite care can take place in your own home, at day-care centers, or at residential or nursing facilities that offer overnight stays. Whether it's for just a few hours a week or an extended vacation, seeking respite care can help ease the burden of family caregiving and help to relieve stress, restore your energy, and promote balance in your life.

Our People

Nurses (RNs), Registered Practical Nurses (RPNs) and Personal Support Workers (PSWs)
Access Personal Support (APS) nurses, registered practical nurses and personal support workers specialize in providing home health care services to seniors and survivors of all ages with acquired brain injuries and/or mental health issues. Access also specializes in providing services to individuals with spinal cord injuries and/or complex needs.

 

Our highly skilled workers possess at least 3 years of community support service experience. They also have a Nursing Degree, Diploma or Personal Support Worker certificate (or equivalent) and are certified in First Aid and Cardiac Pulmonary Resuscitation (CPR), Crisis Prevention Intervention or Safe Management, Safe Food Handling and Foot Care.
